TRIAL FORM QUESTIONNAIRE
- Please give 3 preferred dates/times in case your coach needs to reschedule.
CVC will schedule your trial lesson according to your preferences.
- Submit singing video link 48 hours before first trial lesson.
SPECIAL OFFER:
Provide five (5) email addresses (using the link below) of any singer that you believe would be interested in voice lessons and could improve with CVC.
Once done, we will send an invitation to book a trial lesson with a
20% discount attached.
**Reschedule Request(s) are only accepted 48 hours before your session.
The CVC Team has up to 30 days to reschedule your trial lesson. This varies depending on coaches schedule.
***On the day of your session, after a 10 minute absence, with no written correspondence to email cvc@jbclarke.com, your trial lesson will be forfeited with no refund or ability to reschedule. Additionally, you will not be guaranteed to book another trial lesson for 90 days.
